Records, Payments, and Insurance

Copies of pertinent medical records are available to the patient or an authorized representative for a nominal fee after we receive a signed HIPAA Release. Please allow 15 working days notice. Our office provides a standard return to work/off work form(s) generated at CCSS. Any Special documentation requests other than insurance company requirements, such as letters, disability forms and FMLA forms will be completed for a fee. Please speak with our staff for details.


Our fees are based upon the reasonable and customary charges prevailing in this area and take into account the complexity of a particular problem. Your co-payment will be collected at the time of your visit and may be paid by debit card, credit card, check or cash. Please review our financial policies.


We participate with most insurance plans. Health insurance companies may pay only part of the physician’s fee and services. The extent of your benefits is determined by your individual policy. If a referral is required, it must be presented before you can be seen by our office. 

For planned surgery, our office will contact your insurance company for pre-certification of medical necessity. For major expenses such as emergency surgery or elective surgery, we will work with you to ensure that you are able to obtain all benefits that are due you from your insurance company.
